Units: Rehab, Memory Care, Ventilator, Neurobehavioral, Long-Term Care
Status: Part Time or Full Time
Shift: Days
Rate: $18.50/hr.-$28/hr.

Responsibilities:
•Assists in promoting quality of life for our residents.
•Assists residents with all ADL’s.
•Takes vital signs and weights and reports to RN/LPN team leader.
•Ambulates, lifts and transfers resident as per individual care plans.
•Assists with feeding residents, tray distribution and recording of intakes per policy.
•Interacts and fosters a positive relationship with staff, residents, and families consistently projecting a positive, caring, courteous and responsive environment.
•Observes and reports resident needs, abnormal conditions and/or behavioral changes.


•Successful completion of NYS approved Nursing Assistant program
•Current NYS Nurse Aide certification in good standing
